A Checklist of group health plan coverage transparency information that health plans and health insurers must disclose to plan participants and beneficiaries, using an internet-based self-service tool or in paper form (if requested). The required coverage transparency disclosures reflect a statutory requirement added under the Affordable Care Act (ACA) in 2010 and implemented in tri-agency final regulations issued in November 2020.
