A form that can be completed by an employee to give notice of their entitlement to take statutory paternity leave and pay on the birth of a child. Completion will satisfy the relevant statutory notice and evidential requirements. This document reflects changes which take effect in relation to children whose expected week of childbirth begins after 6 April 2024.
