A Standard Document that is an advance directive for health care for an individual residing in Alabama. A competent adult residing in Alabama may execute an advance directive for health care in the form of a living will or health care proxy, or both. An advance directive protects the individual's right to either refuse or request medical treatment if the individual is incapacitated.
