A form for employees to request a reasonable accommodation for a disability under the Americans with Disabilities Act (ADA), including exemption requests from a COVID-19 mandatory vaccination policy. This form can also be used by an employer to document an employee's oral request for a reasonable accommodation. It can be attached to and referred to in the employer's disability accommodations policy. This Standard Document applies only to private workplaces. It is based on federal law. State or local law may impose additional or different requirements, but this document is useful and relevant to employers in every state.
