A Standard Document used to revoke an existing power of attorney. This revocation of power of attorney terminates all powers given to the agent under an identified power of attorney. This revocation of power of attorney should not be used to revoke a medical power of attorney, which is a power of attorney authorizing an agent to make health care decisions.
