A Standard Document allowing an individual residing in Maryland to authorize another individual, known as a health care agent, to make health care decisions and receive protected health information on the individual's behalf. The document also allows the individual to provide instructions for specific health care treatments the individual would like to receive if the individual is in an end-of-life medical condition, including directives regarding the use of life-sustaining treatments or end-of-life care.
