Employee Vaccination Certification Form | Practical Law
An employee certification form that employers can use to track employees' COVID-19 vaccination status either as a stand-alone protocol or in conjunction with a mandatory or voluntary vaccination policy. Employers can use this as an employee self-certification or acknowledgment, an employer verification, or both.
