A questionnaire for plaintiff-side attorneys to provide their clients to identify relevant evidence, particularly electronically stored information, in the client's possession. This questionnaire assists counsel in identifying the devices (including phones, laptops, desktops, and tablets) and social media, messaging, and email accounts used by the client. It will assist counsel in ensuring that relevant information is preserved and the appropriate devices and accounts are searched during discovery. This document is based on federal law.
