This Checklist addresses agency and employee notice requirements, certifications, and agency action when responding to FMLA leave requests. This Checklist only applies to federal civil service employees covered by Title II of the FMLA and subject to Office of Personnel Management (OPM) regulations found at 5 C.F.R. § 630.1201.
