A Chapter 11 Individual Debtor Questionnaire in the form of a memorandum from bankruptcy counsel to a Chapter 11 individual debtor client. This questionnaire requests the preliminary information required to commence a Chapter 11 case for an individual debtor. Bankruptcy counsel can use the completed questionnaire to prepare the Chapter 11 petition and bankruptcy schedules and assess whether the debtor qualifies as a small business debtor eligible to elect treatment as a small business case or under Subchapter V of Chapter 11 enacted as part of the Small Business Reorganization Act (SBRA).
