Fair Work Information Statement
A document providing employees with information about their conditions of employment, compiled by the Fair Work Ombudsman (FWO) in accordance with Division 12 of Part 2-2 of the Fair Work Act 2009 (Cth) (FW Act).
The Fair Work Information Statement must be given to every national system employee before, or as soon as practicable after, the employee starts employment (section 125(1), FW Act).
An employer's failure to provide the Fair Work Information Statement is a breach of a civil remedy provision (section 44, FW Act).
