A Standard Document used to revoke an existing power of attorney in New Jersey. This revocation of power of attorney terminates all powers given to the agent under an identified power of attorney. This revocation of power of attorney should not be used to revoke an advance directive for health care, which is a power of attorney authorizing an agent to make health care decisions.
