This Standard Document is an advance directive for health care for an individual domiciled in Georgia. It authorizes an agent to make health care decisions for the individual if the individual is unable to do so. It also provides the agent with personal instructions that express preferences regarding particular health care treatments, including directives for the withholding or withdrawing of life sustaining treatments, as well as decisions regarding organ donation and the disposition of the individual's remains.
