A Standard Document creating a directive to physicians (also known as a living will) in Texas. It allows an individual to give instructions for the withholding, withdrawing, or continued administration of life-sustaining treatment if the individual has a terminal or irreversible condition. This Standard Document also allows the individual to express wishes regarding religious or moral considerations related to treatment.
