Individually Identifiable Health Information (IIHI)
In the context of the Health Insurance Portability and Accountability Act of 1996, information that:
- Relates to:
- an individual's past, present, or future physical or mental health or condition;
- an individual's provision of health care; or
- past, present, or future payment for the provision of health care to an individual.
- Identifies the individual or that there is a reasonable basis to believe that it can be used to identify the individual.
- Is created or received by a health care provider, health plan, employer, or health care clearinghouse.
