A form for employers to request medical information from an employer's healthcare provider in response to an employee's request for a reasonable accommodation for a disability under the Americans with Disabilities Act (ADA). This form should be used only if the employee's disability or need for accommodation is not obvious and the employee has not already provided adequate information to confirm the employee's disability or need for an accommodation. This Standard Document applies only to private workplaces. It is based on federal law. State or local law may impose additional or different requirements, but this document is useful and relevant to employers in every state.
