A form that can be completed by an employee wishing to choose a period of statutory paternity leave and pay in relation to the adoption of a child. Completion will satisfy the statutory notice and evidential requirements. This document reflects changes which take effect in respect of children whose placement for adoption is on or after 6 April 2024.
