A form that can be completed by an employee to give notice of their entitlement to take paternity leave and pay on the adoption of a child. Completion will satisfy the relevant statutory notice and evidential requirements. This document reflects changes which take effect in relation to children whose expected adoption placement date is on or after 6 April 2024.
