A Standard Document that is an advance health care directive, also referred to as a durable power of attorney for health care, allowing an individual residing in New Hampshire to authorize an agent to make health care decisions for the individual. This document also allows the individual to provide the agent with personal instructions that express the individual's preferences regarding health care treatments.
